A cyclist was hit by a car on Meath Street at around 4.35pm on Wednesday 4 September.
The vehicle has then collided with a house before fleeing the area.
Officers swiftly attended along with paramedics and a 40-year-old man was taken to James Cook University hospital with a serious injury to his leg.
An investigation is currently underway to establish the circumstances surrounding the collision and to identify the vehicle and the driver.
Detective Chief Inspector John Bonner said: “We are already following numerous lines of enquiry including conducting house-to-house to speak to residents in the area and reviewing CCTV and dash cam footage.
“I am keen to hear from members of the public who were driving in the area at around the time the collision took place and have dash cam footage inside their vehicles. The same goes for residents who have not yet spoken to officers and may also have cars with dash cam footage.
“I would also encourage the driver of the vehicle to contact police.”
Anyone with information is asked to call Cleveland Police on 101 quoting 170824. Information can also be provided anonymously by contacting Crimestoppers on 0800 555 111.
